You will have received notification of  the NHS Summary Care Record.

If you have any questions or if you wish to discuss your options or concerns, please phone the dedicated NHS Care Records Service information line on 0845 603 8510 (local rates apply) or visit the website at www.nhscarerecords.nhs.uk/summary.

Your GP may not have much detailed information about the summary care record at this stage, so please use the information sources if you have more queries see below.  However, your GP can record your wish not to have a summary care record, if that is what you decide.
